Anything that you pay for that saves the government money by not using state services should be paid out of gross income.

I agree. Why should we pay extra for healthcare just because we cannot access the NHS

I agree,
A few years back my wife had some symptoms that she was very worried about (after switching birth control due to the GP changing it without consultation), she tried to go to the GP and was told she could be seen in 2 weeks.
Because we couldnt wait that long we went private where they found the issue immediately, changed her back onto the BC she was on before and no more issues. For the insurance we needed the GP practice to sign off a form.
After getting them to do so they charged us £30 for the administrative time.
Even though we had paid already to be seen privately, we had to pay again for not taking up their time in 2 weeks. On top of that the GP had caused the issue in the first place.
Definitely feel that we are disincentivised from moving costs off the NHS.
